My money is on a release similar to the last couple of George A. Romero films and other low-budget movies with a big name/brand recognition attached: the movie plays at a few festivals, followed by a small theatrical release in the US (and maybe the UK depending on distributor) covering only a few major cities, maybe fifty screens total. The DVD/download/Netflix then comes hot on the heels of the cinema release, perhaps only a couple of weeks later. Basically, I suspect it will have a very small release in indie/larger multiplex cinemas just to generate word-of-mouth for the DVD.
So the movie might not be truly "direct to video", but if it comes to a cinema near you, you'd better be fast to catch it! This is just a hunch, though.

It's possible it will be put out by a company like Magnet Releasing. Coscarelli seemed to have a good experience with them on John Dies At the End so I could see a similar release to that.
